Prospective Home Clinics

Home Clinics commit to Manitoba’s Home Clinic Criteria, and are registered with Manitoba Health.

Shared Health will administer aspects of the Home Clinic program on behalf of Manitoba Health.

Are you Home Clinic Ready?

Registering with Manitoba Health as a Home Clinic is a prerequisite to patient enrolment and, for fee-for-service clinics, to submission of claims for applicable Family Medicine Tariffs. There are a number of actions your primary care clinic can take to improve your readiness, and our Home Clinic team is here to help.

1. Define your Home Clinic

2. Understand the Primary Care Quality Indicators and Data Extract

  • Discuss the Primary Care Quality Indicators (PCQIs) with your practice team and determine workflow changes necessary to support their implementation and submission via the Primary Care Data Extract (PCDE).
  • If you use an EMR certified against the Primary Care Quality Indicator Reminders and Data Extract specification:
    • Learn about clinical alerts and reminders available in your EMR that support patient care in alignment with Manitoba’s PCQIs
    • Determine if there is any configuration, mapping, etc. needed within your EMR product to ensure that your EMR data accurately populates the PCDE
    • Speak with your EMR product vendor to understand how to generate and submit your PCDE
  • If you use a non-Certified EMR or an EMR that has not certified against the Primary Care Quality Indicator Reminders and Data Extract specification:
    • Clinical alerts and reminders will not be available
    • Speak with your EMR product vendor to confirm the ability to properly generate the PCDE
    • Submission of your PCDE will need to occur quarterly via an encrypted USB/CD to Manitoba Health; speak with your assigned Home Clinic Liaison for more information.

3. Prepare to enrol patients and claim applicable Family Medicine tariffs

  • Seek guidance from your EMR vendor regarding how to enrol patients within your EMR product.
  • Learn about the patient enrolment methods you could consider and begin analysis of your EMR data.
  • Learn more about the Home Clinic Enrolment Service or using the Primary Care Data Extract for enrolment purposes, including its limitations and how enrolment data submitted via the Primary Care Data Extract will be processed.
  • Fee-for-service family physicians should:
    • develop a clear understanding of the conditions related to the applicable Family Medicine tariffs and identify eligible patient records; and
    • review the resources available on the Manitoba Health website
