RL Patient Safety Event Reporting
RL Equipment (Medical Device) and Product Complaint Reporting – What You Need to Know
RL reporting is being improved for incidents with equipment or supplies as a contributing factor. Product complaint details will also be incorporated into the incident report.
What’s Changing
• Eliminating duplicate documentation with the deletion of a separate product complaint form
• Standardizing collection of key information that supports quicker escalation of patient and staff safety risks
• Improving efficiency of search tables for equipment and supplies
What’s Staying the Same
RL reporting of equipment/medical device incidents that could have or did impact patients will continue in each form. For general equipment or supply issues, you can report under the Facilities/Equipment/Environment icon.
RL is Shared Health’s provincial web-based incident reporting system that captures safety events and public feedback. Online reporting allows our health-care system to:
- Review incidents in real time
- Provide greater visibility to the safety events occurring
- Highlight opportunities for learning and system improvement
- Collect data that helps inform decision making
Having a provincial view allows us to link safety themes across Health Authorities, combine our efforts for improvement, and share our learnings to make health care safer for all Manitobans.
RL Benefits
- Staff are able to report incidents in a consistent way across the province, and have them shared in a timely way with need-to-know team members across the health system. The information entered in incident reports can be used to take action and keep patients and staff safe.
- Managers can update incident “files” and run unit- or program-specific reports on trending issues. This helps focus energy on addressing important quality and safety issues for patients and team members.
- Patients and families benefit from near real-time reporting and the ability of the health system to be responsive in understanding safety issues and improving services. This supports our goals and vision of providing the best patient care possible.
Refreshing RL Safety Event Forms
