Update 24/7 Contact Information

Full and accurate information pertaining to physician identification and location is critical for Shared Health to provide timely results for your patients, especially when critical test results require urgent communication.

This information is also essential for Shared Health’s compliance with the College of Physicians and Surgeons of Manitoba’s (CPSM) By-Law #11, Standards of Practice in Medicine, Coverage – Critical Test Results, as well as for meeting our own accreditation requirements.

Update the Shared Health Service Desk of any changes to your address, phone/fax number and pertinent practice information by completing the NEW/Amended Physician Information and Fax Confirmation Sheet (F10-50-26C) and forwarding it to:

All Laboratory requisitions that have an asterisk (required field) beside the Critical Results phone number have tests that can produce a critical result, and the lab requires an after hours contact number on the requisition or in our Lab Information System (LIS).
