Pathway for Referral
The following resources will facilitate assessment of ongoing post COVID-19 condition symptoms and their impact on daily function. Based on the assessed severity and impact of symptoms, these resources provide guidance to appropriate referral pathways for patients with moderate to severe ongoing symptoms or to Long COVID services available to patients with mild to moderate symptoms without a health-care provider referral.
COVID-19 Yorkshire Rehab Screen (C19-YRS)-Manitoba
The COVID-19 Yorkshire Rehab Screen has been adapted for use in Manitoba to assess patients for presence and level of impact of symptoms across several categories including:

Provider Post COVID-19 Condition Referral Pathways
Provider Post COVID-19 Condition Referral Pathways provides direction for further referral based on the patient’s ongoing symptoms of post COVID-19 condition. Recommendations for pre-referral workup are provided along with contact information for referrals.

Public Long COVID Services
Public Long COVID Services/Private Providers lists resources and services available without a prescriber referral. This document can be shared with patients and/or families to guide further supported self-management. While some resources are available at no cost for Manitobans, many are fee-for-service. The private service providers included on this document have shared awareness of their Long COVID programs but this list is not considered all inclusive. The private providers are listed as a starting point for patients and/or families to access targeted services. The occupational therapy and physiotherapy private practice directories are linked to the document if the patient or family is interested in a different service provider.
