What is the difference between an EMR, EPR and EHR?

An Electronic Medical Record (EMR) is an individual’s medical record in digital format used within primary and specialist care and may be customized to the clinic’s requirements. An EMR can be described as a provider-centric or health organization-centric health record of a person, where personal health information can be entered (e.g. doctor’s orders) into the EMR by authorized health care providers.

An Electronic Patient Record (EPR) is a facility-based (Acute or Long Term Care) record containing detailed patient health information including identification, allergies and test results that can be used facility-wide for the person’s care, and may contain one or more sections or ‘modules’. An EPR can be described as a provider-centric or health organization-centric health record of a person, where personal health information can be entered (e.g. doctor’s orders) into the EPR by authorized health care providers.

The electronic patient record (EPR) is made up of various modules, such as:

  • Provincial ADT system
  • Computerized physician order entry (CPOE), results viewing
  • Emergency department information system (EDIS)
  • Scheduling
  • Clinical documentation

An Electronic Health Record (EHR) is a secure and private lifetime record of a person’s key health history, available to be viewed electronically by authorized health care providers at the right time at authorized sites in support of patient care. An EHR is not a detailed record of every health-care episode, and is often search and view only. An EHR is a person-centric health record (versus a health-care provider-centric record). EChart Manitoba is our provincial EHR.

Downtime

What should you do if you suspect that the eHealth_hub service is down?

  • Refer to your clinic’s procedure on how to receive results if the eHealth_hub or EMR is down
  • Determine if:
    a. there is only one result missing in your EMR system,
    b. there are several results not arriving in your EMR system,
    c. there are no results arriving in your EMR system but they ARE arriving in eChart Manitoba
  • Contact the Service Desk at 204-940-8500 or 1-866-999-9698 to report the problem and your determination of the a, b, c scenarios previously listed
  • Test results may be available for viewing in eChart Manitoba if you are provisioned to use eChart

What is the difference between eHealth_hub and eChart?

eHealth_hub is a Shared Health service that coordinates electronic delivery of information between source systems and authorized site electronic medical records (EMRs) for patients that receive care by the site. As an example, a health-care provider could receive their ordered laboratory results electronically into their EMR instead of by fax. Visit eHealth_hub for more information about this service.

eChart Manitoba is a view only, secure electronic system that allows authorized health-care providers access to patient health information when needed. It pulls together information from many existing systems in Manitoba, including, but not limited to, patient demographic information, dispensed and public health medications, lab results, immunizations, diagnostic imaging reports, primary care enrolment and Home Clinic Client Summaries and select hospital encounters.

How are results routed from the source to the EMR?

Each source’s system is technologically different and routes information according to the unique features of its Laboratory Information System (LIS) or Radiology Information System (RIS). Results are routed to the ordering provider(s) and/or location(s) as well as “Copy To” provider(s) and/or location(s) as indicated on the requisition.

Diagnostic Imaging Reports:

  • Reports do not include diagnostic images. DI reports will be routed to the ordering location indicated on the lab requisition.
  • DI reports identified as “Copy to” will be routed to a single-location mailbox, using a DIST-assigned identifier specific to each provider and location.

How frequently are results uploaded from eHealth_hub to my EMR?

The upload of results from eHealth_hub to your EMR is dependent on the type of EMR at your clinic and the settings that are programmed within it. For more information about the frequency of your EMR electronic results uploading, contact your EMR vendor or your local IT support.

How can my clinic go paperless?

Diagnostic Services, Shared Health (including Winnipeg, Westman and rural labs) and Cadham Provincial Laboratory have the ability to turn off paper delivery of certain laboratory results. The results that are not delivered electronically will continue to be delivered on paper.

The Radiology Information System cannot turn paper off.

Imaging

Do all diagnostic imaging reports completed in the province display in eHealth_hub?

Diagnostic image reports from Manitoba’s Radiology Information System (RIS) have been available from November 3, 2011 onward. This includes diagnostic image reports from hospitals and other publicly funded facilities from across the 5 Regional Health Authorities, but excludes image reports from sites located within the regions previously known as Brandon RHA, Assiniboine RHA and any private facilities that currently use different types of RIS systems other than the provincial RIS system.

Only diagnostic image reports available in RIS are sent via eHealth_hub. For example, echocardiography/ echocardiograms are not included in RIS and therefore will not be sent to your EMR.

Note: As only the MRI RIS system at Pan Am clinic is connected with the provincial RIS system, only MRI reports from Pan Am Clinic will be sent.

What modalities and facilities are available for DI reports in eHealth_hub?

Here is a list of Diagnostic Imaging reports available in eHealth_hub from facilities and modalities.

What types of DI exams from Manitoba’s RIS are available through eHealth_hub?

Diagnostic Imaging refers to any type of procedure performed by a radiologist or a radiology technologist and done with the following modalities:

  • Angiography (AN)
  • Computed Tomography (CT)
  • Fluoroscopy (FL)
  • Magnetic Resonance Imaging (MR)
  • Nuclear Medicine (NM)
  • Positron Emission Tomography (PT)
  • Radiography
    • X-ray (XR)
    • Mammography (MA)
    • Bone Mineral Densitometry (BD)
    • NOTE: BD not included in eHealth_hub since RIS does not get reports for these.
  • Ultrasound (US)
  • Fetal Assessment (FA)

DI exam reports that fall into one of the modalities above can be delivered through eHealth_hub depending on the facility.

Will only completed and dictated tests display in eHealth_Hub?

Only reports which have been dictated, validated, and electronically signed by a Radiologist will display in RIS and be sent to your EMR. Reports will display as “Final” or if an addendum has been done to provide additional clinical information the report will display as “Corrected.”

Note: It is possible that there are multiple addendums within the report, so be sure to note the dates and times of any amendments.

Why do we receive multiple diagnostic imaging reports that contain the same information (duplicate reports)?

Depending upon the location and the type of imaging exam being performed, multiple exam codes may be required by the RIS system to complete one request. This causes the same report to be sent for each exam code.

Laboratory

Why are newborn screening results unmatched in the EMR?

Newborn screening results from Cadham Provincial Laboratory will flow into your Electronic Medical Record (EMR) as unmatched since there is no PHIN provided at birth. Along with other demographics, the Manitoba Family Registration Number (6-digit number) will be sent as an identifier to provide you with additional information to create a patient chart. Infant requisitions sent from Winnipeg hospitals will include the hospital MRN identifier.

Why don’t I see newborn screening results in my EMR?

Only EMRs that receive results from Cadham Provincial Laboratory (CPL) receive newborn screening results. Talk to your EMR Vendor to learn more about receiving CPL results.

What laboratory sources currently provide information to eHealth_hub?

Diagnostic Services, Shared Health:

  • Winnipeg and Westman locations (Nov. 2012 onward)
  • Rural locations – see details in FAQ below (Oct. 2013 onward)

Cadham Provincial Laboritories (Oct. 2019 onward) – Note: not all Manitoba Certified EMRs offer connectivity to Cadham Lab results at this time.

Other Electronic Lab Sources

Why would a clinic want eHealth_hub if we already have a connection to our private lab?

eHealth_hub delivers certain lab information from Diagnostic Services, Shared Health and Cadham Provincial Laboratory.

The private lab feed will only include their own results, so if a patient chooses to go to any other lab, the results will be delivered on paper or scanned efax rather than electronically. eHealth_hub allows clinics a single connection to include other lab and DI sources and therefore receives more results electronically.

Shared Health is working with private labs in the province to have their results included in the single eHealth_hub electronic feed.

Receiving results electronically allows additional EMR functionality to be utilized such as electronic trending and analysis on patient groups. Paper and efax results cannot be used for electronic trending and analysis unless the results are manually keyed into the patient chart. EHealth_hub results are electronic and would contribute to this additional EMR functionality.

eHealth_hub can also deliver digital imaging reports from Manitoba’s Radiology Information System (RIS).

Troubleshooting

How do I report eHealth_hub delivery issues?

You can report receipt of electronic reports for tests that were not ordered by the clinic / provider and failure to receive electronic reports that were ordered by the clinic / provider using the Delivery Exception Report.
