What is Diagnostic Imaging Central Intake?

  • The Central Intake’s mandate is to provide a single point of entry for all non-emergent outpatient diagnostic imaging requisitions. 
  • It serves as a central point of reference for referring clinicians and/or patients on where their requisition for a diagnostic image has been distributed.
  • Central Intake does NOT schedule any apts.  All apt are scheduled at the Diagnostic Imaging site.

What is the process for MRI requisitions?

  • The “Request for Consultation for a Diagnostic Imaging Exam” form (requisition) is completed by a referring clinician and faxed to Central Intake.
  • Requisitions are then faxed to one of the Shared Health diagnostic imaging facilities based on the shortest wait time for the requested exam.
  • The diagnostic imaging site will triage the patient requisition and schedule an appointment.
  • The patient will receive a letter in the mail identifying their appointment time, date and any necessary preparation instructions.
    • Appointments that are required to be scheduled between 1-3 weeks may be notified by telephone.
  • On a daily basis, referring clinicians will receive a faxed list identifying which patients were scheduled for appointments, and their subsequent appointment date and time.

What is the process for CT, Ultrasound and Nuclear Medicine requisitions?

  • The “Request for Consultation for a Diagnostic Imaging Exam” form (requisition) is completed by a referring clinician and faxed to Central Intake for Winnipeg request only, and Winnipeg physicians.
  • Requisitions are then faxed to one of the Shared Health diagnostic imaging facilities based on the shortest wait time for the requested exam.

Or

  • The “Request for Consultation for a Diagnostic Imaging Exam” form (requisition) is completed by a referring clinician and faxed to the desire diagnostic imaging rural location.
  • The diagnostic imaging site will triage the patient requisition and schedule an appointment.
  • The patient will receive a letter in the mail identifying their appointment time, date and any necessary preparation instructions.
  • Appointments that are required to be scheduled between 1-3 weeks may be notified by telephone.

On a daily basis, referring clinicians will receive a faxed list identifying which patients were scheduled for appointments, and their subsequent appointment date and time.

What if I want to go to the shortest wait time in the province?

There is an option on the requisition to indicate “willing to travel”

What if I want my request to go to a specific site?

There is an option on the requisition to indicate the ‘preferred’ site.

Has my apt been booked already?

  • You will receive either a letter in the mail or a phone with your schedule apt information.
  • If you have not received a letter or a phone you can contact central intake to find out which site you requisition was distributed too

What do if I need to reschedule my appointment?

  • If you need to reschedule your appointment, please contact the diagnostic imaging site your appointment was scheduled at.
  • Central intake does NOT reschedule appointments.

Can an ordering physician send in a requisition for greater than a year from now?

Central intake does NOT accept requisition that are requested for greater than 365 days in the future.

Can I be put on a cancellation list for my test?

Central intake does NOT manage the cancellation list. You will have to contact the site your requisition was distributed too and ask to be put on the cancellation list.

Does Diagnostic Imaging Central Intake process endoscopy/colonoscopy requests?

No.

Where can I find the report for my exam?

  • Your doctor should receive the report via fax once it has been finalized.
  • Your doctor can also access your report on e-Chart.
  • You can also contact the site where your exam was performed and request a copy.
