Social prescribing: Exploring community-led approaches to improve health systems (QI Power Hour)
Social prescribing is a model that enables health care providers and social service professionals to connect individuals with non-clinical supports and community resources that address individual and community needs based on the social determinants of health. Social prescribing is an emerging practice across over 20 countries around the world and has more recently gained grassroots momentum across Canada.
In this webinar participants will:
