The Virtual Medicine Ward, based at HSC, provides hospital at home to patients who can be supported in the comfort of their own home. The ward facilitates interventions and monitoring that ensures safe care while recovering from illness in a familiar environment.

Building on a previous models of remote care, the Virtual Medicine Ward provides hospital-level care within the comfort of a patient’s home.
This service leverages technology to support up to 30 admitted patients across Winnipeg. By providing essential treatments such as IV therapy, and oxygen, the VMW ensures patients receive the care they need from a dedicated multidisciplinary team of health care professionals.
The Virtual Medicine Ward prioritizes patient safety and quality care with a goal of optimizing patient flow across Winnipeg’s hospital facilities.
Criteria
If you suspect your patient can be virtually monitored by a team of nurses, physicians, paramedics for home visits and pharmacists, please consider the Virtual Ward.
Referral and Transfer to the Virtual Ward
A physician to physician consultation is required for patients to be admitted to the virtual ward.
Contact the Virtual Ward physician via TigerConnect or HSC paging. Navigate to the roles section and search for ‘Virtual Ward-Medicine-Physician’.
The admission of a patient to the virtual ward should be considered as a transfer from their current service area to the virtual ward. Virtual ward patients are and should be thought of as admitted patients.
Once a patient is accepted, follow these steps to transfer your patient to the Virtual Ward:
- Retrieve a Virtual Ward Kit (Transfer package will be enclosed)
- Follow the steps outlined in the transfer package
