RL Updates
An RL software update being implemented on October 6, 2026 will introduce key changes to improve transparency and support incident management processes. These changes will enhance visibility and support effective incident follow-up.
What you can expect to see
The biggest changes most staff can expect to see are:
- New login screen
- The introduction of Basic Employee RL accounts
- File submission tracker
New Login Screen
Currently, there are three different ways to log into RL, which can create confusion and is a risk as users may submit an RL incorrectly, or think they are submitting an RL anonymously when they aren’t.
The new RL screen fixes this.
From the new RL screen, users will have three options:
- Log in with User Name and Password. This option is only to be used by Affiliate sites, or users without a network account.
- Submit anonymously. Click this button if you want to submit your RL event anonymously.
- Login with Single Sign-On. Click this will create or log you into your basic employee account. Network users will be redirected to Multifactor Authentication after clicking on this button. More information on basic employee accounts can be found below.

Affiliate Site Login
Affiliate sites will use this section to log in. Network users who try logging in here will get an error message.
Submit Anonymously
Click here if you wish to submit an anonymous incident.
Sign into your Basic Employee Account
Users can create and sign into their Basic Employee Account here.
Basic Employee RL Accounts
Employees can create a basic RL account by clicking on the “Login with Single Sign-On” button on the login screen.
Basic Employee Accounts allows submitters to:
- Save incomplete RL submissions and complete them at a later time. This is a new feature that staff have requested through system feedback. Incomplete submissions will be saved for 30 days.
- Find out an update or outcome(s) on their submissions through File Submission Tracker.
Adding Basic Employee Accounts improves patient safety culture by providing staff with increased visibility and transparency on incident follow-up. Staff can now see what actions file managers are taking to improve patient safety as a result of their incident reporting.
File Submission Tracker
The File Submission Trackers allows staff to view the status of incident reports they have submitted. Staff can see if an incident they have submitted it: New, In Progress, or Closed. They can also see what outcome actions were taken.
