Paramedic Referral to Social Prescribing

A few things to remember:

• Each zone will have some variation in services offered
• Ensure that you have selected the appropriate zone that your patient resides in
• Ensure that the patient is aware that there may be some costs associated with some services, they are marked with an * on the form
• Ask if they would like to be part of a brief survey on Social Prescribing
• Leave an information pamphlet with your patient
• If the patient is not sure about being referred tell them that they can call the intake line or email the address on the pamphlet if they change their mind at any time

After this form is submitted, it will be forwarded to the resource coordinator in their community and patients should expect a phone call to discuss what they would like assistance with.
