eHealth_Hub Provider Update

This form must be submitted when a provider is added to/removed from a clinic. Failure to report a provider change will result in delivery disruption of clinical results.

For ownership, location, network or vendor changes, a Private Clinic Change form must be submitted a minimum of four (4) weeks before the effective date.
