Disclosure of Patient Safety Events – Being Open with Patients & Families workshop

The goal of this interactive workshop is to enhance participants’ skill and comfort level to disclose unexpected or adverse patient outcomes. Effective and compassionate disclosure provides an opportunity to re-establish trust and rapport in the face of these outcomes.

This workshop will assist clinical team members to put regional policy
Patient Safety Events: Management and Disclosure of Occurrences, Near Misses and Critical Incidents
(10.50.020) and national disclosure guidelines into practice. A unified regional approach to disclosure aligns
with our commitment to patient centered care and patient engagement strategies. The workshop will focus on
disclosure requirements for occurrences and Critical Incidents. We will be reviewing relevant clinical case
studies as a group, allowing time for practice and discussion. This workshop will be practical and collaborative
– delivered by members of the WRHA Quality, Patient Safety, and Accreditation team.

Learning Objectives
#1. Describe the disclosure process for all types of patient safety events
#2. Identify barriers to disclosure
#3. Integrate the C-O-N-E-S model into disclosure conversations
#4. Complete the disclosure process following all patient safety events
